Primary Health Choice, Inc. offers quality services that include Targeted MH/SA Case Management, School-Based Therapy, Intensive In-Home, North Carolina Innovation Waiver, Medication Management, Telemedicine, and Developmental Therapy, Personal Assistance, Respite B3, Individual, Group Therapy, and Family Therapy. The agency provides these services specific to best practices as identified by the NC Medicaid and Health Choice Clinical Coverage Policies 8A, 8C, 8L and 8P. Intensive In Home

The Intensive In-Home (IIH) service is a team approach designed to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions.

Medication Management & Psychiatric

Medication management is the prescription, use, and review of psychotropic medication with no more than minimal psychotherapy. Medication management is used when evaluating the effects of prescribed psychotropic drugs, with only minimal psychotherapy provided.

Comprehensive Clinical Assessments

A comprehensive clinical assessment is an intensive clinical and functional face-to-face evaluation of a beneficiary’s presenting mental health, developmental disability, and substance use disorder.

Outpatient Therapy

Outpatient behavioral health services are psychiatric and biopsychosocial assessment, medication management, individual, group, and family therapies, psychotherapy for crisis, and psychological testing for eligible beneficiaries.

NC Innovations

The North Carolina Innovations Wavier Services (NC Innovations) is a resource for funding services and supports for Medicaid beneficiaries with intellectual and other related development disabilities who are at risk for institutional care in an Intermediate Care Facility for Individuals with Intellectual Disabilities.

Respite B3

Respite services provide periodic support and relief to the primary caregivers from the responsibility and stress of caring for children (ages three (3) to twenty-one (21)) with mental health, developmental disabilities and substance abuse, and for adults with developmental disabilities.

Developmental Therapy

Developmental Therapy is a developmental disability service that includes individually designed instruction, training or functional developmental intervention activities based on the assessment of and unique strengths and needs of the individual child or adult.

Personal Assistance

Personal Assistance is an IPRS Funded IDD service that provides support, supervision, and engaging an individual in activities of daily living. This service is used to assist in maintaining skilled learned through habilitation services. Utilized during waiting period for approved waiver services.

Diagnostic Assessments

A Diagnostic Assessment is an intensive clinical and functional face-to-face evaluation of a beneficiary’s mental health, intellectual and developmental disability, or substance use condition. The assessment results in the issuance of a Diagnostic Assessment report with a recommendation regarding whether the beneficiary meets target population criteria and includes a recommendation for Enhanced Benefit services that provides the basis for the development of the PCP.
